To make the most out of your time we ask that you print off (or pick up) our intake form, complete it to the best of your ability, and return it to our office at least 1 day prior to your appointment. Your first visit will last for about an hour and consists of an in-depth consultation and condition specific examination. We not only address what current concerns you have, but begin to tailor a program for your personal well-being. Please bring in any recent laboratory results pertinent to your concerns.
